Information & Forms for New Students

Welcome! The links listed below connect to important communications and forms that you will need to review or complete in order to assure your student status at Macalester.

Please read the general information letter first as it outlines everything. You are REQUIRED to complete and return a health history, have a physical by a physician, and send in your immunization records indicating that you have had all required immunizations. US students must also have had a TB test; international students will have a TB test done here at campus.

Note that during the second half of May, all of June and July, we are only staffed one day a week. Staff return full-time August 4th. We will be in moving to our new location in the Leonard Center that week and open for business on Monday, August 11. We encourage you to email any questions that you may have to: healthservices@macalester.edu. We will respond as quickly as possible.
