Off-Campus Study Proposal

Deadline for Off-Campus Study in Fall 2013 or Spring 2014 is February 26, 2013 at 4:30 p.m. Please note that these forms must be typed.

Instructions:

1. Complete and print the Off-Campus Study Proposal
   This proposal must be typed. Sign where indicated. Meet with your academic advisor to discuss your Four Year Plan. Your academic advisor signs the Advisor Confirmation.
2. Download Essay Instructions
   Write the essays required for your off-campus plans. All students must write Essay A; many students need to write additional essays. Read the instructions to know more.
3. Coursework Approval Form(s)
   Complete for any courses for which you would like to receive major, minor, or concentration credit. Meet with the chair of that academic department. The chair decides what courses will/may/will not count and signs the form.
4. Make copies.
   Make two copies of each original form - the Proposal and the Coursework Approval Form(s) - and two copies of your essays. Then, create stapled packets with one copy of each form and one copy of each essay. The forms should go in this order: Proposal, Coursework Approval Form(s), Essay(s). You will then have 3 complete packets (original + 2 copies).

Change of Program/Deferral Form

Fill out this form if there is any change in your off-campus study plans (such as deferral, program change, etc.) and you still intend to study abroad. This must be communicated promptly and in writing to the International Center.

Cancellation/Withdrawal Form

Fill out this form if you decide not to study off-campus (i.e. cancel or withdraw from a program). This must be communicated promptly and in writing to the International Center.
