Registration Forms

• Registration Deadlines
  Information deadlines and requirements for Summer 2013 and Fall 2013.
• Complete and bring these three items to the Internship Office to register your internship.
  • 1. Learning Contract
    See Developing the Learning Contract for helpful tips.
  • 2. Student/Site Internship Agreement
    This is a 2-sided form requiring signatures from both student and site supervisor.
  • 3. Position Description Template
    Use if the organization/company hasn't already provided you with a position description.
• Summer/January Additional Documentation Required by the Registrar
  1. Summer/January Registration Form (signed by faculty, department chair, and Internship Program)
  2. Student and Parent Waiver (required only for internships outside of the Twin Cities)
• Add/Drop slip
  Available in the Registrar's Office or the Internship Program. We will sign off on the add/drop form when you have submitted the above three documents.

• Intent to Register an Internship
  Need more time to complete the paperwork for your fall internship? Turn in this form by 9/14/12 to extend the registration deadline to 9/21/12.
• Sign up for a Career Exploreship
  Sophomores may use this form to sign up for a January informational interview, one-day job shadow, or three-day job shadow with alumni hosts working in the Twin Cities; Washington, D.C.; or New York City.Check back in November 2013 for details.
