Documentation

Students requesting accommodations or services must provide appropriate documentation of their disability. Please see the specific documentation guidelines required for certain disabilities. If you have a question regarding the required documentation or need information about a disability which is not listed, please contact the Associate Dean of Students at (651) 696-6220.

In general, all documentation must include these elements:

  • The credentials of the evaluator
  • A diagnostice statement identifying the disability
  • A description of the diagnostice methodology used
  • A description of the current functional limitations
  • A description of the expected progession or stability of the disability
  • A description of current and past accommodations, services and/or medications
  • Recommendations for accommodations, adaptive devices, assistive services, compensatory strategies, and/or collateral support services

Accommodations can only be made with appropriate documentation and following a meeting with the Associate Dean of Students. Students are encouraged to meet with the Associate Dean within the 1st two weeks of each semester to ensure timely accommodations. Any accommodations or services provided will be in effect only after final documentation review of the disability is completed. Grades which have already been posted on exams, papers, projects or in overall classes cannot be changed.

We have compiled a list of psychiatrists, doctors, and counselors close to campus for assistance with diagnosis, overseeing medication, and longer term support.
