Forms and Surveys
This page includes brief descriptions of several tools that are available to web page maintainers at Macalester College. This is not a comprehensive list of every available form and survey tool. Please contact Web Services and we would be happy to help you select the most appropriate tool for your needs.

form-to-email (macform script)
Data from a web form is sent via e-mail to designated recipients. Each time the form is submitted, the server automatically sends an e-mail message listing the field names in the form and the corresponding data values. Recipient sees individual e-mail messages, one for each form submission. Subject line for the e-mail and list of recipients can be specified by the author of the web form using hidden form fields. The Template Wizard includes an option to generate a simple web form that uses this form-to-email script.

Includes unique tracking number for each form submission. Includes option for a confirmation e-mail to be sent to the person who fills out the form.

form-to-database (Google Forms)
A web form is developed using the "Form" option in Google Docs. The form can be embedded in an existing web page, or linked to via a link generated by Google Docs. Each time the form is submitted, the server saves the submitted form data into a Google Spreadsheet file. The data file can be viewed at any time using Google Docs. To get started, open Google Docs, select the New pull-down menu, and click on Form. Help for forms is available as part of the Google Docs online help system.

form-to-database (Web Services custom)
This option involves custom configuration by Web Services staff and can be set up in a number of ways. The form can be developed by the page maintainer or by Web Services. Each time the form is submitted, the server can do one more of these actions: do validation checks on form fields to make sure data is submitted in a particular format; send an e-mail message listing the field names in the form and the corresponding data values; send a confirmation e-mail message to the person who filled out the form; save the submitted form data in a database; redirect the browser to a different web page.
survey tool (SurveyMonkey)
SurveyMonkey is a hosted survey system that supports survey creation, data collection, and reporting of results. A basic account is free but is limited to surveys of 10 questions or less and 100 or fewer responses. For longer surveys with larger target audiences, a "Pro" account is available. Self-service access to a Pro account is available through SurveyMonkey on a month-to-month basis for a reasonable fee. Web Services has an annual Pro account; we can work with departments to create and launch surveys using our SurveyMonkey account. A survey created with a basic account can be transferred to our account, enabling users to get started with the survey development on their own. Once the survey is created, we will provide you with a link; you send the link to the selected population for your survey. We also provide you with a link to view and analyze your survey results. For student projects, requests must be made by a faculty sponsor.
