A PDF will be sent via email to the Macalester Employee, Supervisor and Accounts Payable once completed and signed.<\/li>\n\n\n\n<li><strong>Attach the completed Missing Receipt Declaration<\/strong>&nbsp;(that will be sent as a PDF once the Supervisor has signed ) when a receipt is missing and add to the&nbsp;&nbsp;<strong>Check Request<\/strong>.<\/li>\n\n\n\n<li>For federal grants,&nbsp;ALL&nbsp;receipts must be submitted, regardless of dollar amount.<\/li>\n<\/ul>\n\n\n\n<p>Please contact Accounts Payable at&nbsp;<a href=\"mailto:AccountsPayable@macalester.edu\">AccountsPayable@macalester.edu<\/a>&nbsp;or&nbsp;(651) 696-6565&nbsp;with any questions.<\/p>","protected":false},"excerpt":{"rendered":"<p>Complete the\u00a0Accounts Payable Missing Receipt Declaration (eForm)\u00a0whenever a receipt for a transaction of\u00a0$50\u00a0or more is lost or otherwise unavailable and all measures to obtain a copy have been exhausted.
