Incoming students will need to add required immunization data and complete required forms in the Macalester Patient Portal. You can do so by following these instructions.<\/p>\n\n\n\n

1. Log into the patient portal<\/h2>\n\n\n\n

Follow this link, https:\/\/macalester.medicatconnect.com\/<\/a>, and log in using your Macalester credentials. <\/p>\n\n\n\n

2. From the patient portal homepage, navigate to “immunizations”<\/h2>\n\n\n\n

3. From the Immunization tab, click on the “required” dropdown and add each required immunization. <\/h2>\n\n\n\n

In compliance with Minnesota state law, Macalester College requires these immunizations<\/a>. <\/p>\n\n\n\n

Select both the vaccination type, and the date it was administered.<\/p>\n\n\n\n

Click submit to save your record. <\/p>\n\n\n\n

4. Complete your required forms.<\/h2>\n\n\n\n

Navigate to forms in the main menu.<\/p>\n\n\n\n

Complete both the informed consent and TB Screening Questionnaire. <\/p>\n\n\n\n

If you answer yes to any of the TB Screening questions, you will also need to complete the TB Screening Assessment, which will need to be completed by a medical provider. You MUST complete a Tuberculin Skin Test (TST) OR a Interferon Gamma Release Assay (IGRA); AND a Chest x-ray IF TST or IGRA is positive.<\/strong><\/p>\n\n\n\n

Upload your completed Assessment to uploads. <\/p>\n\n\n\n
