This is not a test. There are no trick questions and answers will have meaning only for you. To get the most out of the assissment , answer honestly about where you are now.

After each statement choose the number that identifies your response in the radio button after each question

The chart below serves as a guide for determining your score from each wellness dimension. There is no perfect chart and no right balance – it has to be what works for you.

Intellectual Wellness Developing and utilizing one's knowledge and creativity. This includes: applying what one's learned, developing lifelong learning and imporving skills.
Physics Wellness Taking good care of one's body. This includes: exercise, nutrition,sleeping, sexual health, and awareness of substance and chemical intake.
Emotional Wellness Awareness, acceptance and expression of one's feelings. This includes: stress management, positive sense of self, coping mechanisms, taking responsibility for your behavior and determining and adhering to boundaries.
Social Wellness One's ability to participate effictively and positively with the surrouding community. This includes: tolerance and acceptance, developing relationships, time management, respect of others, multi-cultural competence and concern for community welfare.
Spiritual Wellness Exploring value and meaning in one's life. This includes: hope and optimism, developing a set of personal values and ethics, having a sense of purpose, caring for others and acting out of that care and making time for self-reflection.
Environmental Wellness Engaging in the surrounding environment and developing health habits to enhance it. This includes: conservation, sustainable practices, safety and awareness of issues and concerns in the surrounding environment.
Financial Wellness Assessing and managing the resources required to accomplish one's goals. This includes: money managment, time management, differentiating between necessities and luxuries and planning for the future.
Occupational Wellness Finding enrichment in one's life through work. This includes: balance of work and leisure, accepting responsibilities and following through, having a positive attitude towards work.
